New VFIS Airway Verification form available for download
The new downloadable airway verification form is a means of documenting advanced airway procedures performed in the prehospital setting.
The form is designed to be utilized in the field to document the procedure. The final and critical aspect of the form is to obtain a signature of the receiving physician verifying proper placement of the advanced airway prior to leaving the receiving facility. The form may then be copied for the receiving facility. The form may also be utilized as a reference when completing the prehospital patient care record by the advanced life support provider.
